Connecting individuals to benefits and services provided by other networks
TCHD works with many other health and human services organizations. The Community Health Worker team can assist community members with finding and connecting to other resources to get support for all of their different, personal needs. Individuals can learn more about local resources and services available through:
- Teton County Network of Care Services Directory -- look up local services and learn more information on your own
- Connect with CHOW drop-in hours at Teton County Library -- visit with a CHW to work together on finding the resources that fit your situation
